Answer these questions and complete your plan documents in 15 minutes or less.

Beneficiaries

Who will be your primary and back-up beneficiaries? You can have more than one. If more than one, they will divide the assets equally. This excludes any specific gifts you make.

Specific Gifts of Property

Will you leave specific gifs of real estate or personal property? If yes, make a list of specific gifts. Include a description of the property and the full name of the beneficiary. You do not have to do this if you want everything to go to your primary beneficiary.

Guardianship

Who will care for your minor children? Identify a primary and back-up. It can be more than one. Make sure you have the conversation with them before including them in your plan documents.

Executor / Personal Representative

Who will handle the affairs of the estate? Name a primary and back-up to handle the businsess of your estate.

Financial Power of Attorney

Who do you trust to handle financial matters if you're unable to, for any reason? You'll only need to name one person.

Health Care Power of Attorney

Who will be your health care agent? Name a primary and back-up health care agent to ensure your health care desires are carried out if you are unable to do so due to illness or incapacity.
